A 14 year old female, studying 9th class R/O Narketpally was brought to casualty with CHIEF COMPLAINTS: Shortness of breath   Fever  Rash over the abdomen since 10 days HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 2 days back,  then she developed SOB, sudden in onset. No H/O chest pain, Pedal edema, palpitations, excessive sweating, giddiness and wheeze Fever since 2 days, high grade, a/w chills and rigor since 2 pm afternoon, relieved on medication. Rash over the abdomen since 10days, no itching . Started as vesicles filled with watery fluid. On applying RMP prescribed ointment, vesicles turned white and now became red resolved macules like. This also reflects my patient centered online learning portfolio. A 55 year old male patient, resident of Yadadri came with CHIEF COMPLAINTS   : Pain in abdomen since 2 days. HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 8 days back, then he developed pain in epigastric region Associated with 2 episodes of vomitings, which has food particles as content. Pain was sudden in onset, gradually progressive, intermittent and dragging type. Pain was aggravated with alcohol intake. Burning sensation is also felt over peri umbilical region. Patient initailly started drinking and smoking from 30 years . This also reflects my patient centered online learning portfolio. A 55 year old male patient R/O Thirumalagiri came to OPD  CHIEF COMPLAINTS - 1. Tingling sensation in Right upper and lower limbs since 4 days 2. Weakness in the Right upper and lower limbs since 4 days HISTORY OF PRESENTING ILLNESS- Patient was apparently asymptomatic 4 days back. Then he developed weakness of Right upper and lower limbs which is insidious in onset, gradually progressive. Weakness is associated with tingling sensation which is improving    No history of deviation of mouth , loss of consciousness, headache, giddiness, vomitings,fever,SOB, pain.  No history of involuntary passage of urine or stools   Slipping of chappals is seen while walking.
