Hello, I am Harshitha Shalini , a 5th semester medical student.     This is an online elog book to discuss our patients health data after taking his/her consent. A 60yr old female came with CHIEF COMPLAINTS: - fever since 10 days -productive  cough since 10 days -vomitings since 10days HOPI: Patient was aparently asymptomatic 10 days back.She then developed fever low grade,intermittent ,not associated with chills and rigors ,relieved on medications C/o cough with expectoration since 10 days ,whitish mucoid sputum not blood tinged C/o nausea and vomitings since 10days 2- 3 episodes per day which is watery ,non projectile ,non blood tinged with food particles as contents C/o constipation and decreased appetite since 10 days C/o pain in the back while coughing  No H/O Burning micturition,loose stools,pain abdomen No h/o pedal edema,chest pain, facial puffiness,decreased urine output,SOB, palpitations  PAST HISTORY: No simil

A 60 year old female with facial puffiness, pedal oedema and knee joint pains

Hello, I am Harshitha Shalini , a 5th semester medical student.     This is an online elog book to discuss our patients health data after taking his/her consent. This also reflects my patient centered online learning portfolio. A 60 year old female, who was an agricultural labourer by occupation (but has stopped working since 4 years), came to the hospital with CHIEF COMPLAINTS - pain in both knees since 4-5 years, facial swelling since 2-3 months, pedal edema since 1 week. HOPI- The patient was apparently asymptomatic 4-5 years ago. She then started developing pain in both her knees which is aggravated on walking and standing, relieved on resting and medications (painkillers). She stopped working due to the pains. 2-3 months ago, she developed facial puffiness which was insidious in onset and gradually progressive in nature. A 33 year old male with headache and stiffness of joints

A 33yr old male patient labour by occupation came to OPD with   CHIEF COMPLAINTS: Headache(on and off) since 1yr and stiffness of joints. HOPI: Patient of apparently asymptomatic 1yr back then he developed headache of unilateral side(Right) with full aching pain -also had complaints of joint stiffness on sitting for a long time. Negative history: no photophobia, no watering of eyes,no blurring of vision,no vomitings. A 55 year old male with SOB and pedal oedema

A 55 year  old male patient resident of  madhavagudem came to OPD with  CHIEF COMPLAINTS - -SOB since 15 days -pedal edema since 1 week HOPI: Patient was apparently asymptomatic 15days back then he developed SOB (grade 1and2) that was  insidious in onset, gradually progressive  C/o pedal edema since 5 years and aggravated since  last 15 days it is a pitting type C/o decreased urine output since 1week No C/O chest pain, palpitations, orthopnea, PND. No H/O fever, burning micturition, loose stools, vomiting.  No C/O pain abdomen. 75 year old female with altered sensorium

A 75yr old female was brought to the casualty with CHIEF COMPLAINTS: Drowsiness and giddiness since 16/07/23 afternoon HOPI: patient was apparently asymptomatic till 2pm, after which she started feeling drowsy and sleepy. She took 2 tablets of ranitidine at 2pm. She is drowsy but arousable. No h/o altered behaviour, agitation, irrelevant talk, screaming.
